Abstract

U radu su opisane mogućnosti primjene bioimpedancijskih izmjera u medicinskoj dijagnostici te je dat sažet pregled danas upotrebljavanih metoda i postupaka. Posebno su prikazane mogućnosti bioimpedancijske dijagnostike kod ishemičkih oboljenja donjih ekstremiteta te pojave sindroma mišićnih odjeljaka. U uvodnom dijelu prikazana je struktura biološkog tkiva. Prikazana su električka svojstva skeletnih mišića, definirana kompleksnom vodljivošću i kompleksnom permitivnoću, kao i temeljna saznanja o bioelektričkoj impedanciji. Objašnjeno je također međudjelovanje električke struje i biološkog tkiva. Nadalje je razmatran utjecaj prelazne impedancije između biološkog tkiva i elektroničke mjerne opreme, te je na osnovu rezultata istraživanja predložena nova metoda uklanjanja tog utjecaja primjenom dvoelektrodne mjerne tehnike na više frekvencija. U svrhu pronalaženja što točnijeg i jednostavnijeg načina mjerenja biološke impedancije mišića potkoljenice izvršena je usporedbena analiza dvoelektrodnog i četveroelektrodnog mjerenja te su istaknute prednosti i mane obje mjerne metode. Da bi se pokazala mogućnost primjene dvoelektrodnog multifrekvencijskog mjerenja bioimpedancije mišića potkoljenice u prognostičko-dijagnostičkim mogućnostima pri donošenju odluke o kirurškoj revaskularizaciji donjih ekstremiteta izvršena su ispitivanja na oba ekstremiteta u 51 bolesnika sa unilateralnom cirkulacijskom insuficijencijom. Provedena je normizacija mjernih postupaka i dat je pregled mogućih uzroka mjerne nesigurnosti prilikom mjerenja bioimpedancije. Temeljem rezultata mjerenja i usporedbe s vrijednostima tkivnog tlaka određeno je frekvencijski interesantno područje u dijagnostičke svrhe kao i ovisnost modula i faznog kuta biološke impedancije o stupnju ishemije u donjim ekstremitetima. Opisan je novi (FEEC) električki model biološkog tkiva te su iznijeta poboljšanja novog modela u odnosu na do sada upotrebljavane modele. Dat je način proračuna i optimizacije parametara nadomjesnih električkih modela nakon čega se pristupilo analiziranju i usporedbi mjerenih i proračunatih podataka. Izvršena je usporedba vrijednosti parametara novog nadomjesnog modela i stvarnog stanja tkiva te su naznačene mogućnosti primjene ovog modela u dijagnosticiranju sindroma mišićnih odjeljaka. S ciljem što jednostavnije i brže analize mjerenih podataka, izrađena je baza podataka i druge programska podrška potrebna za automatizaciju mjernog postupka. Na kraju rada dat je kraći opis novorazvijenog uređaja, projektiranog za mjerenje biološke impedancije tkiva s ciljem dijagnosticiranja sindroma mišićnih odjeljaka.

Abstract (english)

In the work, different possibilities of bioimpedance measurements use in medical diagnostics are discussed and short survey of contemporary methods and procedures is described. The emphasis is put on the possibilities of bioimpedance diagnostics of ischaemic diseases in lower extremities, particularly on the manifestation of the muscle compartmental syndrome. In the introduction, the structure of biological tissue is described. The electrical properties of skeletal muscles, defined by a complex conductivity and a complex permittivity as well as the basic conception of bioimpedance, are described. The interaction of the electric current and biological tissue has been also explained. Furthermore, the effect of the electrode impedance between the biological tissue and the electronic measuring equipment is discussed. On the basis of the measurements, new method of electrode impedance elimination by use of multifrequency bipolar measuring technique is suggested. Trying to achieve precise and simple way of measuring lower leg muscles bioimpedance, a comparative analysis of bipolar and tetrapolar measuring techniques has been performed emphasizing the advantages and disadvantages of both methods. In order to prove the diagnostic possibilities of lower leg’s muscles, bipolar multifrequency bioimpedance analysis measurements of both extremities on 51 patients with unilateral circulation insufficiency were performed. After standardizing measuring procedures, the summary of possible reasons for the measurement uncertainty is given. Based on the measurement results and comparison of tissue pressure, useful frequency range for diagnostic purposes has been determined. A new electrical model of biological tissue (FEEC) has been developed and the improvements of this new model in comparison with the former ones are discussed. The calculation and optimization of electrical model parameters has been shown, accompanied by analysis and comparison of measured and calculated data. Comparison of parameter values of this model and the real tissue condition has been made and the possibilities of using this model in diagnostics of muscle compartmental syndrome are presented. In order to enable analysis of the measured data to be simpler and faster, database and appropriate software is designed, allowing automatization of the measuring procedure. Finally, the new custom developed equipment, designed for measuring tissue bioimpedance in order to diagnose the compartmental syndrome, is described.
